  • The trader should apply to the Licensing Authority / DHS, seeking Clearance for “Drug License (Retail / Wholesale)”

  • The trader should submit the filled given prescribed format of "Form 19", with required supporting documents to the Controlling Authority / Deputy Director (Medical), DHS, A&N Admn., Port Blair.

  • The Application will be processed by the concerned office for verification, correctness and completeness of the filled in details and the supporting documents enclosed with the application. The site will be inspected by the Drug Inspectors and if all requirements are fullfilled, the trader will be asked to pay the licensing fees, after that the Licensing Authority / DHS will issue the "Drug License" clearance document to the applicant.

The trader has to submit the Application for Drug License in Physical to Controlling Authority / Deputy Director (Medical), DHS, A & N Admin, Port Blair.

For Retail / Wholesale Drug License

  1. Statutory Form – 19 for license

  2. Declaration by the Proprietor/Partner/Director/Competent Persons/Registered Pharmacist with proof of residential address(Present and Permanent) for proof of residential address – Aadhar Card, Pass Port, Driving License, Voter ID

  3. Partnership deed in case of partnership firm/ List of Directors downloaded from MCA website signed by Company Secretary/Managing Director (in case of company).

  4. In case of company an Affidavit under section 34 of Drug and Cosmetics Act, 1940 on Rs.20/- stamp paper signed one of the Directors of the company in prescribed proforma

  5. Declaration by the Registered Pharmacist regarding acceptance of job on Rs.20/- Non- Judicial stamp paper in prescribed proforma duly signed by Public Notary

  6. Declaration by the Proprietor regarding full time engagement of Pharmacist on Rs.20/- Non- Judicial stamp paper in prescribed proforma duly signed by Public Notary

  7. Declaration by the Proprietor-cum-Pharmacist on Rs.20/- Non- Judicial stamp paper in prescribed proforma duly signed by Public Notary. (If the Pharmacist himself is a proprietor)

  8. Declaration by the Proprietor regarding non-conviction under Drugs & Cosmetics Act & Rules on Rs.20/- Non- Judicial stamp paper in prescribed proforma duly signed by Public Notary.

  9. Declaration by the owner of the premises regarding No Objection for conducting the business of Retail drugs on Rs.20/- Non- Judicial stamp paper in prescribed proforma duly signed by Public Notary.

  10. Self attested copy of Registered Pharmacist certificate (renewal up to date) affixed with latest original photograph and signature of the candidate /original to be produced to the Drug Inspector at the time of inspection for endorsement

  11. Self attested copy of qualification certificate of the Registered Pharmacist

  12. Plan of the proposed premises indicating the carpet area (specifying length and breadth in meters and area in sqm) with the signature of owner of the premises and the applicant (Pro/partner/Authorized signatory/Managing Director, etc) in a legal size paper

  13. Declaration by owner of premises (Photograph of the building owner with self attestation) in prescribed proforma. Self-attested photocopy of the document showing the proof of ownership of the premises to be licensed (Legal / Revenue records showing the present ownership)

  14. Copy of the Rent / Lease deed in case of rental premises

  15. Invoice of the newly purchased Refrigerator in the name of shop / proprietor

  1. Area of the outlet to verify the compliance with the statutory limits (Minimum 100sq.feet for Retail / Wholesale) and (Minimum 150sq.feet for both)

  2. Capabilities to provide good storage conditions of the drugs stocked in the premises such as racks, refrigerator (for cold storage drugs) etc

  3. Premises should be RCC structure and located on ground floor for Retail shop

The Licensing Authority / Director of Health Services, A & N Admin will issue the Drug License clearance to the trader.

Not Available

  • The applicant shall apply for Registration/Renewal in Prescribed format (Form A) in duplicate along with the supporting documents copies and bond paper

  • Acknowledgement slip shall be issued by the Appropriate Authority on verification if all documents are not enclosed

  • Application for fresh registration/renewal should be disposed off within 70 days from the date of receipt of application

  • Inspection date to be fixed & the inspection committee comprising of HOD Gynaecologist, Radiologist, Member Secretary (PC&PNDT), Deputy Secretary (Law), Chairman Social Welfare Board shall inspect the premises – adequate spare for examination of pregnant women, waiting area, toilet facility for pregnant women etc. Hoarding of PC&PNDT to be placed

  • The committee shall provide the inspection report-satisfied and agreed to issue NOC for purchase of USG machine.

  • The member secretary (PC&PNDT) shall give permission for purchase of Ultra Sound Machine

  • After purchasing the USG machine the applicant shall intimate the Appropriate Authority for inspection of the machine

  • The inspection team shall inspect the machine & verify the modal, make etc of the USG machine

  • Inspection report to be submitted to the Member Secretary and necessary registration certificate in Form B and H shall be issued to the applicant

  • Any change in the USG machine, staff etc shall be intimated to the appropriate authority & necessary amendment to be made in Form H.

  • The applicant shall apply for renewal of registration 30 days prior to its expiry date in Format A to the Appropriate Authority

  • Note: - No fees for Govt. Institutions

  • If the application is rejected and the applicant resubmit it within 90 days – no fresh Demand Draft required & if applies after 90 days a fresh Demand Draft of Rs.25000/- (Rupees Twenty Five Thousand Only) to be submitted.

  • For New Registration of USG Clinic 90 Days from Date of Receipt of Application, subject to condition that the application form is complete and correct, all required supporting documents are enclosed and satisfactory observations of Inspection Committee on physical inspection of the site

  • For Renewal of USG Clinic 30 Days from Date of Receipt of Application, subject to condition that the application form is complete and correct, all required supporting documents are enclosed and satisfactory observations of Inspection Committee on physical inspection of the site

  1. Genetic Counselling Centre: Such laboratory should have or acquire such of the following equipments as may be necessary for carrying out chromosomal studies, bio-chemical studies and molecular studies.

      18. Such other equipments as may be necessary.

  2. Genetic Clinic/ultrasound clinic/ imaging centre should have or acquire such of the following equipments as may be necessary for carrying out the tests or procedures –

    1. Equipment and accessories necessary for carrying out clinical examination by an obstetrician or gynaecologist.

    2. An ultra – Sonography machine including mobile ultrasound machine, imaging or any other equipment capable of conducting foetal ultrasonography

    3. Appropriate catheters and equipment for carrying out chorionic villi aspirations per vagina or per abdomen

    4. Appropriate sterile needles for amniocentesis or cordocentesis.

    5. A suitable foetoscope with appropriate accessories for foetoscopy, foetal skin or organ biopsy or foetal blood sampling shall be optional

    6. Equipment for dry and wet sterilization

    7. Equipment for carrying out emergency procedures such as evacuation of uterus or resuscitation in case of need

    8. Genetic work station.
